Accountable / Named GP for All Patients

All patients registered at St Martin's Medical Centre have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

How to register

You are welcome to join the practice, provided you reside within the practice boundaries. According to the practice policy in order to register we require 1 proof of ID and 2 proof of current address (within 3 -6 months). We will offer you an appointment to have a health check which is a part of your registration.  Please bring details of any medication you are taking.

Children 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

Submitting your registration form [GMS1/2] at reception does not automatically guarantee a registration with the practice, you will receive a call in due course, from the surgery to confirm your registration with St. Martin’s.
