How to register
The doctors welcome new patients who live within our practice area. Please check with reception to see if your address falls within our catchment area.
Please complete this simple New Patient Registration form
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a new patient health check with the nurse.

Disabled Patient Facilities
Disabled patient facilities are also available at this practice.
Parking
No Parking for patients-There is ample parking available on the side roads, the main road (please check times), and the Hillingdon Council car park.
If you have disabled badges you may be able to park on the road outside the surgery.
